Case: Obstetric trauma in third trimester

A 29-year-old female who is at 30/40 gestation has been involved in a 60km/hr MVA. She is complaining of lower abdo/pelvic pain. Her vital signs are as follows:

RR 22
Sats 96% RA
HR 128
BP 95/60
GCS 15

1. List four key features of the obstetric examination in this patient. 4 marks

Bloods have been sent for FBC, EUC, VBG and police BAL.

2. List four other initial investigations you will perform in the ED and provide a justification for each. 4 marks

The patient remains tachycardic despite volume resuscitation and analgesia. CT imaging is being considered. The patient and her partner are concerned about radiation exposure.

3. Summarise the advantages and disadvantages of CT in obstetric trauma. 4 marks
